Working at Scale has become a key driver across general practice. ‘At Scale’ provision of care, will increasingly see practices shift towards collaborative working models, formal mergers, partnerships and federations and ICSs. But, what are the practicalities of working at scale'?

- The different options of working at scale – Soft Merger, Full Merger, Federations

- Why merge? – Benefits / Perceived Risks

-  Practicalities of merging – Key workstreams

-  How can we do this and when will we get the time

-  To be model – What will we look like after we have merged

-  Federations – Different models

- Federations – Benefits and challenges

- ICP/ICS/LCO/LCS – Primary Care – Integrated delivery

Enhancing the patient experience

Active Signposting (AS) is to help patients access the right care at the earliest opportunity. It is a new system of ‘triage‘, carried out at the first point of contact with the GP Practice by non-clinical staff under direction of the clinical team.

 

- Delegates will understand each of the core skills areas

- They understand the benefits of supporting and enabling healthier behaviours and self-care for patients.

- They will gain an understanding of how to work across a range of service providers

- Will understand the potential complexities and risks.

 

Successful and safe Active Signposting is reliant on 3 core principles:

  • Robust agreed protocols.
  • A full commitment from the whole team to support staff in delivering Active Signposting.
  • Continuing audit of activity and outcomes including complaints and significant or serious incidents.

1.40 – 1.55pm   Practice Management Through Change – The PMs Panel

The session will give participants new insight into the practice managers driving change. A collection of ‘tour de force PMs’, will share their insights… and contribute to a general Q&A.
